Post-Physical Therapy

The post-physical therapy program is designed for any individual that has completed inpatient or outpatient therapy with a licensed physical therapist. The participating individual must have a release form from their therapist or their physician that includes any exercise recommendations or restrictions. Individuals will be screened for program compatibility and will be informed of the benefits and the risk factors associated with participation.
The exercise based program is intended to provide a continuation of the medical based rehabilitation that was initiated in the hospital or in the outpatient clinic. The goal of the program is to continue the patient’s physical recovery through a monitored exercise program, using established exercise protocol and a format that is similar to personal training. Along with the intent of improving the rehabilitation outcome from the original injury or event, the program will also focus on preventing the reoccurrence of injury. The program will also allow the individual to build the foundation of a regular exercise program, or will suggest redirection or modification of programs for those already involved in a consistent exercise regimen.
The post-rehabilitation therapy program will include an initial evaluation of the participant’s needs and goals based on the existing level of recovery and based upon the input of the therapy professional. An individualized plan of exercise will be designed that will include one-hour sessions, three times a week, with a trainer specializing in post-therapy programs. Programs are either 4 week, 8 week, or 12 week sessions determined by the participant’s current level of recovery, motivation level, existing knowledge of exercise, and other health factors.
