Cardiac and Pulmonary Program

(Post-Rehabilitation)

Program Qualifications:
This program is designed for individuals with cardiac or pulmonary disease who have successfully completed therapy in a supervised medical setting and who wish to continue a monitored program. Participants must have a physician’s release to enroll in the program. The release should include any medical recommendations or restrictions for exercise. Individuals will be screened for program compatibility and will be informed of the risk factors associated with participation.
Program Purpose:
The purpose of the cardiac and pulmonary program is to provide an option for individuals to continue an exercise program that will have similarities to the medical supervised rehabilitation they have successfully completed. The post-rehabilitation program will have the goal of improving clinical outcomes by continuing to enhance the recovery from an event or diagnosis, and will also have a goal of preventing a secondary occurrence, especially in regard to heart patients. The program is designed to provide continued motivation to the individual for lifestyle and health improvement, while providing a safe exercise environment. The final goal would be to transition the individual to a regular exercise program and to incorporate learned lifestyle modifications into normal daily living.
Program Description:
The program is supervised by an exercise professional who will monitor the individual’s program, who will promote healthy behaviors and lifestyle modification, and who will motivate the participant to take the necessary steps to restore functional capacity. The program includes consultation with a registered dietitian and a monitored exercise regimen that continues the restoration process that was initiated in a medically supervised environment. The program offers monitoring reports that the participant can provide to their physician.
