Skilled Nursing & Long-Term Care Program
(Inpatient Care and Services)

Inpatient skilled and long-term care services are designed for any individual that needs 24-hour nursing care and supervision. Skilled services are available for those individuals who are medically stable but require highly skilled nursing services. For admission, individuals must have a physician’s order for skilled nursing care, must exhibit a need for a secure and safe environment, and must not be a threat or harm to self or others. The program requires that the individual currently be under the direct care and supervision of a primary care physician.
The purpose of inpatient care is to provide a safe and secure environment where quality of life can be maintained at the highest level for as long as possible. The involvement, interaction, and intervention of the medical and facility staff includes not only caring for the individual’s daily needs and assistance with the activities of daily living, it also involves developing a plan of care specific to the individual. This care plan is based on acuity level and developed through interaction with the resident and the resident’s family. Skilled nursing care typically involves specific short-term goals that require an interdisciplinary approach to improve an individual’s condition of health. Skilled services, therefore, may include such things as physical, speech, and occupational therapy, I.V. therapies, advanced wound care, pain management, enteral feeding, and other more complex nursing services or rehabilitation.
Skilled and long-term care services include 24-hour nursing care, oversight with dietary needs, rehabilitation services, assistance with activities of daily living, and various other nursing and medical services. In addition to medical and nutritional services, individuals are offered daily activities, spiritual care, and are encouraged to maintain social interaction and involvement. Staff members are trained specifically for elder care and have experience with aging issues and understand the aging process. Family involvement is encouraged and family support services are available.
