Diabetes Education


Program Qualifications:
This program is designed for any individual, family member, or care provider of any individual who has been diagnosed with diabetes. The participant must have an interest in learning about diabetes and should be willing to make necessary lifestyle modifications to improve self-management of the disease.
Program Purpose:
This program utilizes a set of educational tools that have been developed by Healthy Interactions, Inc. and the American Diabetes Association. The Merck Journey for Control™ program has sponsored the patient education tools used in this instructional format. These tools, or U.S. Diabetes Conversation Maps®, are designed to allow participants to explore health facts about diabetes through instruction, interaction, and dialogue. The purpose of the session is to improve the health and quality of life of the participant through education, to attempt to reduce or delay the onset of complications from the disease, and to achieve better outcomes in managing diabetes through knowledge and through peer support and motivation.
Program Description:
With the use of the U.S. Diabetes Conversation Maps®, the instructor leads small groups through the components of the program. The Maps are designed to encourage open discussion about diabetes, with the intent of educating the participants and allowing them to ask questions about what they are most interested in learning about the disease. The program is led by a Registered Dietitian (RD) or another educator that has been certified to teach the program and to utilize the Conversation Maps®.
